Once you have reviewed the instructions to proceed to online forms
We would like to thank you for taking part in our process of accepting athletic participation paper work online. This automated process will allow us to be more efficient in handling the forms as well as saving valuable resources. These forms are required each year your son or daughter wishes to participate in athletics for HMS. The Athletic Department would like to thank you and your son/daughter for filling out the forms and submitting them in a timely manner.

Students are required to use the Pre-participation – Physical Examination Form provided. NO OTHER PHYSICAL EXAM FORM WILL BE ACCEPTED. A new physical is required for all 7th - 12th grade students, prior to the start of the new school year. Physicals dated prior to April 1st will not be accepted.

1. The required annual athletic physical and health history cannot be completed online because it must be completed by the parent/guardian and physician. You may fill out and print a physical and medical history by selecting the “Physical/ Medical History” tab at the top of the page. Once you have selected the tab you will see the link to the physical and medical history and will be able to download and print. You are encouraged to fill out the medical history prior to printing the form.

2. All other required paperwork will be done electronically. The parent/guardian will be asked to provide information such as phone numbers, email addresses, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically. You must have your son/daughter’s Student ID number in order to fill out the required forms.

3. Remember that to submit online appropriately, you and your child must complete and sign in all required spaces. Forms will not be accepted electronically without all required information being completed.

4. To access the required athletic participation forms for HMS, hold your cursor over the “Electronic Participation Forms” tab. You will get a drop list of all required forms.

5. To begin, click on the form name and fill out the information requested. Parent/Guardian must have their son/daughters present along with the students ID number available to complete the forms. If you do not know the answer to a question, please answer the question with N/A, or None.

6. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create and “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an IPAD, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus.

7. Once you have filled out all of the information on each page, you will have the opportunity to print the document.

8. You will receive a confirmation email once the document(s) have been reviewed by the HMS Athletic Training Staff.
